Tuesday afternoon and during the normal office hours by appointment Monday thru Friday.

Birth
and Death Certificates
Certified Birth and Death Certificates may be acquired through the Health Department either in person at the Kingston office,
by mail, or on this
website here. A separate application must be used for each individual. The cost of Birth Certificates is $15.00 for each certificate ordered. The cost of Death Certificates is $13.00 for the first certificate, and $10.00 for each additional one ordered at the same time. Death certificates are available beginning from 1980 and birth certificates are available beginning from 1920. For certificates prior to these dates, please contact the Missouri Department of Health and Senior Services. Include with the application form a self addressed stamped envelope, a clear copy of a drivers license or other photo identification, and a check payable to Caldwell County Health Department. An additional $1.50 will be charged for mail service.

PRIVACY STATEMENT
The CCHD is required by law to maintain the privacy of your health information and to provide to you and your representative this Notice of its duties and privacy
practices. The CCHD is required to abide by the terms of this Notice and may be amended from time to time. The CCHD reserves the right to change the terms of this Notice and make the new Notice provisions effective for all health information that it maintains. If the CCHD changes its Notice, the CCHD will provide a copy of its revised Notice to you and your appointed representative. You or your personal representative have the right to express complaints to the CCHD and to the Secretary of this DHHS if you or your representative believe that your privacy rights have been violated. Any complaints to the CCHD should be made in writing to the administrator. The CCHD encourages you to express any concerns you may have regarding the privacy of your information. You will not be retaliated against in any way for filing a complaint.
